i-THRIV Wellness, Delivered – Independent Contractor Agreement & Provider Terms
Effective Date: July 17, 2025
1. Overview & Acceptance
By accepting bookings or working with i-THRIV Wellness, Delivered (“i-THRIV,” “we,” “us,” or “our”), you agree to follow all provider policies, appearance standards, client protocols, and legal obligations outlined in this agreement. You acknowledge that you are an independent contractor (1099) and not an employee of i-THRIV.
2. Dress Code & Appearance
All providers must wear all-black professional attire (black top and black pants)
No shorts, crop tops, revealing, or unprofessional clothing
No personal or competing business logos
i-THRIV branded shirts are available for purchase
3. Equipment & Setup Standards
Arrival Time: Arrive 10–30 minutes early depending on service for setup
Providers must bring all necessary equipment:
Massage/Facial Providers: Clean massage table or facial setup, fresh linens
Sheets: white, green, blue, or black
Covers/blankets: white, green, blue, or black
Yoga & Sound Bath Providers: Must bring yoga mats and sound equipment
Equipment must be in clean and excellent working condition
Bring your cell phone, valid ID, and provider license/certificate
Must have a reliable vehicle and valid liability insurance
4. Conduct & Professionalism
No drinking or drug use before or during sessions
Maintain a clean, quiet, and professional demeanor
Respect client confidentiality and uphold a safe space
Cancel shifts responsibly via the scheduling platform
No unsolicited promotion of personal businesses or tips
5. Independent Contractor Terms (1099)
You are an Independent Contractor, not an employee
You are responsible for:
Paying your own taxes (Form 1099-NEC issued if earning $600+)
Maintaining current licensure and liability insurance
Providing your own tools, products, and transportation
No entitlement to employee benefits (e.g., PTO, unemployment, health insurance)
6. Booking & Platform Policies
All bookings must go through the official i-THRIV system
No cash payments, offline arrangements, or contact info exchanges
Repeated late cancellations or no-shows may result in removal
7. Client Preference, Schedule & Travel Radius
As an independent provider, you have the right to:
Accept or decline clients based on gender comfort (male or female)
Choose your own cities, travel radius, and working hours
We support your wellness and boundaries—do not overwork yourself
Update your availability regularly to avoid double-bookings
8. Social Media Etiquette
If you'd like to share session setup photos, ask clients for permission first
When posting, please tag us using #iTHRIVWellnessDelivered
Always represent the brand with professionalism and respect
9. Non-Solicitation & Brand Protection
Do not hand out business cards or promote personal services during i-THRIV appointments
Attempting to poach clients will result in platform removal
10. Zero-Tolerance Policy
Sexual misconduct, unprofessional behavior, or harassment will lead to immediate termination
Maintain appropriate boundaries—verbal, physical, and digital
11. Client Complaints & Dispute Resolution
In the event of a client complaint, i-THRIV will investigate. You may be contacted to provide your account of the session. Repeated or unresolved complaints may affect your standing.
12. Session Documentation
Providers are encouraged to make brief private session notes (e.g., preferences, allergies, feedback) for quality control and repeat booking purposes.
13. Safety Protocols & Emergency Procedures
If you ever feel unsafe or threatened during a session, you have the right to leave immediately and notify i-THRIV.
14. COVID-19 / Health Protocols
Providers are expected to maintain hygiene standards and follow any state or local health mandates.
15. Use of App or Scheduling Technology
Providers are required to use the i-THRIV app or designated scheduling tool to confirm appointments, update availability, and receive client requests.
16. Rate Acknowledgment & Payment Terms
Payouts are processed within 1–3 business days following a completed session. Rates are fixed per service and confirmed upon booking.
17. Photos, Testimonials, and Media Releases
With your permission, photos or testimonials you provide may be used in marketing materials. No image will be used without client and provider consent.
18. Conflict of Interest
As an independent contractor, you are welcome to work with other companies or platforms, including Zeel, Soothe, or your own business. However, we ask that you be mindful and respectful when managing your schedule.
If you accept a booking from i‑THRIV (e.g., a 4:00 PM session), please do not accept another booking for the same time on a different platform. Always honor the commitment you accepted first—whether it's through i‑THRIV or another source.
We also encourage you to give yourself adequate time between clients and appointments, no matter the platform. This ensures a smooth experience for you and your clients and reflects positively on all the companies you represent.

19. Session Time Expectations
Sessions should begin promptly and run for the full scheduled time unless the client ends it early.
20. Provider Rating System
Clients may rate your service. Consistent positive reviews may qualify you for premium visibility and perks; repeated poor reviews may result in deactivation.
21. Termination & Compliance
i-THRIV reserves the right to suspend or remove any provider due to policy violations
You may be removed for:
Negative reviews
No-shows or lateness
Poor conduct or legal risk
